How does tramadol work?

One of the ironies of medical research is that, quite often, research finds substances or mixtures of chemicals that have a desirable effect - they are an effective treatment for serious medical problems — but no-one can say exactly how or why they work. All that happens is that a grateful public takes the medication or uses the medical device, and enjoys the benefits when they are cured or has a good quality of life when people resume their everyday activities.

This is the case with tramadol. The research shows there is a good analgesic effect, i.e. it controls moderate to severe pain by changing the levels of norepinephrine and serotonin in the central nervous system. Norepinephrine and serotonin are neurotransmitters, i.e. their function is to carry messages from one part of the nervous system to another. So the assumption is that the messages indicating pain in one part of the body are either changed in some way or diverted. However it achieves it effect, the result is that people who have a source of pain somewhere in their body, are less aware of that pain. One word of warning — tramadol does not treat or cure the source of the pain. So when you feel less pain, do not assume that all is well again. A broken bone remains broken and takes time and rest to heal.

What are the rules when taking tramadol?

As with any prescription-only medication, you should always follow the instructions given by your doctor. Simply swallow the tablet or capsule whole with a glass of water. Do not crush the drug to a powder. There is a real risk that, if the dose prescribed does not control the pain, you will just take more. There are real risks if you take too much tramadol, i.e. never take more than 300mg per day. If you do take too high a dose for too long (or you overdose), your body slows down dangerously, i.e. your breathing and heart rate slow. These symptoms are worse if you have been drinking alcohol. Overdose can be fatal if not treated urgently. There are also risks that any powerful pain killer becomes habit-forming. You should not take this drug if you have any problems with addiction.

What are the side effects?

Some people experience drowsiness with reaction times slowing down, so you should not drive or operate any dangerous machinery until you understand how your body reacts to tramadol and you get used to it. The most serious side effect is that some people experience seizures or convulsions. Tell your doctor if you have a seizure disorder such as epilepsy or have had a head injury. It is also very important to tell your doctor if you are taking any other drugs. Seizures are more likely if you are taking antidepressants, or any medication that relaxes your musculature or relieves symptoms of nausea and vomiting.

You must also exercise care if you have any history of mental disorder. People who are depressed have higher rates of suicide when taking tramadol.
